I forlængelse af forrige knæpost om springerknæ kommer her nogle overvejelser:
💉Først og fremmest: binyrebarkhormon/blokader er spild af tid. Research says so🧐
1️⃣Video: en klassiker til at reducere symptomer og/eller give kroppen ro til at komme sig er, at skrue på de parametre man kan i forbindelse med træning: range of motion, belastning, sæt, gentagelser, pauser i mellem sæt, antal dage i mellem træninger, øvelsesvalg osv. Her er eksemplet med range of motion. Mange med knægener oplever smerter ved dybe squats. Det kan der være mange grunde til, men en måde at arbejde rundt omkring problemet på er, ikke at squatte så dybt mens man genoptræner. Man bør være interesseret i at vedligeholde så meget funktion som muligt når man rehabber, så hvis squat er din ting, så bliv endelig ved i det omfang du kan🙃
2️⃣Video: det vi har evidens for i forbindelse med optræning af senevævsproblematikker (i hvert fald i senerne i benene (springerknæ, akillessener etc.)) er TUNG LANGSOM STYRKETRÆNING. Det virker godt! (Vi er ikke helt sikre på hvorfor, men det virker godt!). I videoen er et eksempel med squats. Det som er lidt sjovt (haha😒🤦‍♂️) med specifik optræning af senevæv, som vist på videoen, er, at man tillader at det faktisk godt må gøre RET ONDT når man træner. Det er ikke det samme som, at det godt må gøre ondt når man laver sin sport eller lign. Men den generelle konsensus omkring senevævsrehabilitering er, at det godt må gøre pænt nas undervejs i den specifikke genoptræning, så længe det klinger af igen efter træning.☝️
3️⃣Video: hvis du har ondt i benene, så bliv dygtig til noget andet imens. Tiden går alligevel og kommer ikke tilbage, så i stedet for, at have ondt af dig selv, så kom udover stepperne og dygtiggør dig. Tid er din mest værdifulde resource - hvordan vil du bruge den?⏳

💥At this time, hypomobility of the ipsilateral SCJ was revealed with a caudal mobility assessment while the glenohumeral joint, ribs, cervical spine, and thoracic spine demonstrated normal mobility
💥Caudal grade III SCJ mobilizations were performed, which immediately improved the patient’s within-session active flexion ROM from 135 to 155 degrees and decreased her pain throughout the movement from 4/ 10 to 1/10 on the NPRS
💥This increase in ROM was maintained between sessions as she returned with 0–155 degrees of shoulder flexion at the beginning of visit 8
💥SCJ mobilizations were continued through visit 10 in increasing degrees of shoulder flexion along with a progression of rotator cuff and scapulothoracic strengthening exercises

• A big reason is that playgrounds are created and designed for kids, instead of for humans.
• This creates a scenario where adults feel weird even stepping foot on the playground to do things like hang, swing, climb...it’s perceived as something meant for the kids.

This doesn’t need to be the case. We need physical spaces meant for human play. We need to break the stigma and preconceived notions of what playgrounds are for.

There are places that ‘get’ this concept...for instance the Beach park/Gibbon slackline park in Santa Monica, CA. It is a place where humans of all ages go to play and workout on a variety of structures, from beginner to advanced.
⬇️
And yes, safety should still be respected. In the age of lawsuits, this must be kept in mind. But why don’t adults have more places to even hang, climb, balance or swing on things...these basic features are not inherently dangerous and can be scaled to an individuals ability.
🔑 To tackle the sedentary and movement deficient lifestyles that most adults live, we need more public spaces meant for movement. Movement needs to be fun. Let’s bring play back.

Daily Shoulder mobility is essential for the long term health of your shoulders and to ensure you can get into optimal shoulder positions under load 🏋️🏽‍♂️

The scapula is attached to the thoracic spine (Upper Back), making T-spine mobility important for shoulder biomechanics 🔥
·
Today we are going to focus on thoracic extension, rotation and  extension in an open shoulder stretch!
·
✅ T- Spine rotation in a table top position with your knees stacked under you hips and shoulders stacked over you wrists.
·
Bring one hand behind your head with your elbow pointing to the ground. Exhale as you twist and open up your chest and upper back. Pause at the top between each rep and focus on really feeling the muscles in the upper back firing 🔥
·
✅ Roller Extension, place the foam roller on your mid back with both hands behind your head. Exhale as you extended you upper back while keeping the ribcage down. A great cue is to think of bringing your sternum to the sky ☁️

✅ Wall Shoulder Opener, my personal favorite pose that should be mastered before attempting back bends on the floor.

Inhale as you lift up you chest, exhale as you melt your chest to the ground and extended you upper back 🔑

✅ Dowel shoulder stretch is hands down one of the best exercises for your shoulder mobility 🔥 ·

Putting aside the amazing street food 🥟 🍛🍜🍤🍲, beautiful architecture⛩, and “unique” traffic patterns 🛵, it’s not hard to figure out why. My guess, a MOVEMENT BASED SOCIETY!!!

We spend a lot of time and money trying to master movement through our education, seminars, CEU’s, etc. We all innately move very well and efficiently when born, somewhere along the lines we lose it. We STOP 🛑 moving. These people, probably out of necessity, have not. The results are what is pictured above (yes that woman is elderly, and yes that man is on top of a latter). Not only do they get in this position, but they also come back up! Moving perfectly, efficiently, and with endurance is still hard wired in their nervous systems. A privilege to observe.

Snap, crackle, pop, scratchy, click, grind, noisy, crunchy. Clients will explain their crepitus in many ways. It’s a bit like the ticking clock in the room. You’re not aware it’s there until someone points it out or you become conscious of it, from there it often appears to get louder and more annoying! The relationship with crepitus and pain is still unclear due to no solid evidence.  However what we do so often see is noisy joints with no sign of joint damage. However also based on what we know even if there is joint changes with crepitus then generally we are going to want to expose them to progressive movement and loading to improve function. Either way after ruling out other serious pathology addressing worry and fear behind crepitus is key in restoring confidence in client movement and loading. + or -beliefs around knee noises in a runner for example may be the difference between a good or poor outcome to treatment and return to pain free running .  This is not medical advice.

The quadratus lumborum (QL) is the deepest abdominal muscle. It's located in your lower back on either side of the lumbar spine. It starts at your lowest rib and ends at the top of your pelvis. It's common to have pain here because you use this muscle to sit, stand, and walk. The quadratus lumborum can perform four actions: Lateral flexion of vertebral column, with ipsilateral contraction. Extension of lumbar vertebral column, with bilateral contraction

As a former #highschool #athlete who stayed active long after graduation and having a child, I NEVER would have guessed that a 12 inch #hurdle would give me #anxiety and so much sweat.  #physicaltherapy is like being forced to do a full workout twice a day like I am trying to win a #trackmeet but it's just so I can tie my shoes and wash my own hair.  #heartrate is down, so time to keep going!  KEEP GOING!  KEEP GOING!

It was weird. Not used to how it feels yet. Body not used to it either. Went in hard and had a blood sugar drop ( I  didn't know what it was, I just got super angry and shaky) had to take a break, eat then finish. (I  wasn't training fasted this time.)
But after that it went great. Did all my work, felt strong, no issues. Will have to time training with eating now.
Won't really know a lot more until 2 or 3 more weeks to compare how my body responds.
But I think it's going to be alright.

Step Up your Plank Game using the ValSlide for a fun option to challenge your Core & Shoulder Stability!!! Get into a straight Arm prone plank with one Valslide under each hand.  Start with sliding one arm away from your body in whichever direction you please while stabilizing your body with your other arm and feet.  Then repeat with the other arm. Try for 5-10 slides each hand while maintaining a neutral spine and pelvis.  Careful not to tip over! This is not an exercise I’d attempt if you’re a beginner or someone that isn’t very strong with normal planks. Also... Try this with your feet wide apart first. Slowly creep your feet closer together to make it more challenging. The wider your base of support, the easier it will be.

Nervous System Mobilization
---------- In this video, I have demonstrated three mobilization positions for the sciatic, median and ulnar nerves (I had to leave radial nerve off due to video length-let me know if you are interested in this one).

For each nerve, there is a 'tensioner' position where the nerve is put on tension across it's entire path. In this position, you will likely experience a neural 'stretch', which feels different than a muscle stretch. Tensioners can be held for several seconds and are usually the first line of defense unless the person's pain is too severe.
.
In cases where tensioners cannot be tolerated, start with sliders. Sliders involve placing one end of the nerve on tension while the other end is slack. In the sciatic nerve, for example, when the ankle is plantarflexed (nervous system on slack distally), the head lifts from the ground, which creates nervous system tension in the neck region. Then, the neck and ankle positions switch, which creates the sliding movement of the nerve. Try alternating back and forth 10-15 times and use the technique whenever you are experiencing nerve symptoms (pain, tingling, numbness).

THEN WHY ARE PEOPLE STILL ON THE FLOOR TREATING THEIR IT BAND LIKE TOUGH PIZZA DOUGH?
⁉️Why⁉️: Many feel the need because they have  hip pain, doctor may have diagnosed bursitis. Or the ouside of their knee is clicking, doctor may have diagnosed them with patelofemoral pain syndrome. Etc.

So how do you release tension in the IT Band???
•••°°°YOU DON'T and YOU CAN'T°°°•••
👩🏽‍🎓A study done by Chaudhry (2008) on the deformation of tendonous faciae concluded that it took 925kg to change the length of the IT Band by 1%😲😲😲😲 (just take a look at that monster in the 2nd photo)
▫️This means if a foam roller, Theragun and a human were all working together at once on the IT Band it still wouldn't loosen.
▫️BUT THATS OK cause you need this tension to maintain stability and alignment when we walk, run, change directions etc.
.
🛠THE FIX🛠:
What we can do is temporarily relieve tension in surrounding muscles and also address the true issue. Instead of rolling the tendon, roll the muscles around and under it (vastus lateralis, hammies) and talk to a professional to FIND OUT THE CAUSE of the problem and fix THAT.

Hamstrings: Foundations of Ambulation

Hamstrings are a group of muscles at thr back of the thighs used in walking, running and jumping, squatting, standing and hip hinging. Their primary function is hip extension and knee flexion.

Hamstrings provide acceleration when the hip is extended and bend the knee for ground clearance. The medial (inner) hamstrings rotate the leg inward, and the outer hamstring rotates the leg outward allowing for fine tuning of leg position during walking, landing and pivoting.

Hamstrings comprise of :
- semitendinosis
- semimembranosus
- biceps femoris
They all originate from the ischial tuberosity, insert on the tibia or fibula over thr knee joint, and flex the knee or extend the hip. They are innervated by thr tibial branch of the sciatic nerve.

Due to the fact that the hamstrings are a two joint muscle and affect hip and knee movement, they have a large impact on posture, pelvic control, knee control and are prone to injury when untrained or dysfunctional. They may also expose other areas of the body to injury due to imbalances.

Due the significance if their functions, hamstrings condition will make or break the athletes in running sports as well as how efficiently a person can walk. Well trained hamstrings improve the biomechanics of the whole body by improving posture and making movement efficient.

(My sincerest apologies if I ever said that to you.😬) On the heels of some really amazing colleagues (like @physiodetective) , I accepted the challenge to change that mindset 🧠 , to stop making blanket statements. and to really partner with my patients to figure out the best ways to work them back to their activity of choice - all while trying to keep my bias out of it. 
What a gift 🎁 it is to be able to think critically and help women problem solve their way right back to full function.

I am a BB woman! I have a ting frame and have carried BB with me since 6th grade. I suffer from back pain almost everyday! Due to my BB I have a mild case of Kyphosis and years and years of damage. My doctor, PT, and Breast surgeon have all said the I was cursed and someone as tiny as me should not have been “blessed” with as big of breast that I have been given. I’ve been doing PT for the past 3 months to help re-strengthen my muscles and spine in my upper back and have a breast reduction in 2 months and were taking the max amount as possible! THANK THE LORD! In the mean time, I will continue therapy and my husband has an electrotherapy doohickey thing at home for me to use whenever the pain is to severe.
